Around 15:15, as schools were letting out in the northwest section of the city, an MVA was put out for the 300 block of Lackawanna Street for a vehicle into a building, E11-R1-M603 and Car 7 responded. During the response, dispatch stated they just received a fire alarm for the Jamestown Village Apartments, at the same address, with no specific building given. Car 7 asked for S1(L3) to respond in case it was unrelated.

E11 arrived moments later to find a vehicle smashed through the front lobby of the Jamestown Village Office building and an employee performing CPR on a victim on the front lawn. Car 7 called for another Med Unit (M602) as an arrest was being worked by E11 & M603. A passenger in the vehicle refused treatment & no employees inside the building were injured. R1 & S1 vented the building, secured utilities, and found only superficial damage to the structure.

The driver of the vehicle appeared to have suffered an MI while driving and ran over the curb a half block away missing several poles, civilians, and slammed into the building with his foot still on the accelerator burning the front tires off the vehicle. Employees inside the building rushed outside and pulled the couple from the vehicle and turned the car off but not before the burning tires created a smoke condition inside the building, the fire alarm panel was also damaged in the crash. CPR was started quickly as the employees wife was a CPR Instructor & E11s AED was used, the victim perished later at St Joseph's Medical Center. Crews cleared the scene within an hour after the vehicle was pulled from the building.
